Providence Hospice provides comfort care at the end of life through intensive symptom management, and support of patient/family identified goals of care.  Providence offers
Medicare-Certified hospice care for adults with a life limiting illness and prognosis of
less than six months. The goal is to improve the quality of life for the patient and family.

• Providence Hospice is the only Medicare certified hospice in Anchorage that provides four levels of care
• Our model is family centered community care that focuses on meeting the patient and family goals of care at the end of life
• An RN case manager is assigned to each individual/family receiving care
• We provide 24/7 access to an RN for urgent questions, needs, and support
• One year bereavement support for the family at no additional charge consisting of mailings, memorial services, and grief support groups
• Care is provided in the home by a multidisciplinary team of care providers committed to the Providence mission

• RN case management
• physician oversight, as well as volunteer, chaplain, bereavement, and social services support.

• dementia
• heart disease
• lung disease
• kidney disease
• liver failure
• stroke
