1. Call Providence Admitting to Pre-Register for surgery at (907) 212-3149.
2. If you are having surgery or a procedure that requires anesthesia, it is necessary to come to the preoperative clinic for lab work and/or instructions. There is no appointment needed. Just come between the hours of 8am and 6 pm, 1-3 days prior to your procedure. Any questions call 212-6014.
3. Check with insurance company to get pre-authorization if required.
4. Print and fill out the patient medical history work sheet and bring to the preoperative clinic and give it to the Pre-Op Nurse.
5. If you have any special needs such as diabetes, contact the Doctor for instructions and to manage your medication.
6. Please bring an up-to-date list of all your medicines, vitamins, minerals and supplements with the dose and frequency. If you do not have a list, bring your medicines in a labeled bottle with you.
7. Arrange for a responsible adult to drive you home and stay with you after surgery/procedure. You may ride in a cab ONLY if you are accompanied by another responsible adult (NOT the cab driver).
8. Your surgeon/physician will provide you with your surgery/procedure time. Please arrive at the hospital 2 hours before your surgery or an anesthesia assisted procedure. If you are having conscious sedation or no sedation, please arrive 1 1/2 hours prior.
9. Wear loose comfortable clothing; DO NOT WEAR ANY JEWELRY, MAKEUP, LOTIONS, FINGERNAIL/TOENAIL POLISH, OR PERFUME. Any rings that are too snug to remove should be removed by a jeweler and left at home. Any hidden piercings or studs must also be removed. Be aware, if necessary, your rings may need to be cut off in day surgery if they can not be removed.
