If you have diabetes, you need to take special care of yourself to stay healthy. At Providence Diabetes and Nutrition Center, we’re here for you. With our compassionate and experienced support, you’ll be empowered to take control of your condition. We’ll help you every step of the way—not just to stay healthy, but to live your best life.

We also help people with nutritional counseling for other purposes, such as weight management and people with chronic medical nutrition concerns. Whatever your specific nutrition needs, count on us to help you stay as healthy as you can be.

Our Center offers a Diabetes Self-Management Training Program for adults. We are recognized by the American Diabetes Association, and we partner with you in all aspects of diabetes management, including type 1, type 2 and gestational diabetes.

When you come to Providence Diabetes and Nutrition Center, we’ll talk with you to determine what services are right for you. You may see one or more of our registered nurses, certified diabetes educators, or registered dietitians.

Research shows that people who receive this type of counseling have a lower risk of complications and a better quality of life.

  • Insulin injection training
  • Insulin pump consultation
  • Medical nutrition therapy
  • Personal continuous glucose monitoring training and download
  • Professional continuous glucose monitoring study
Our Medical Nutrition Program

This program offers services such as:

  • Bariatric surgery evaluations
  • Disordered eating counseling
  • Medical nutrition therapy
  • Tube feeding consults
  • Weight management counseling.

We understand that you have a busy life and we want to make your visits with us easy to manage. An important part of your care is insurance coverage. We recommend that you talk with your insurance company about your coverage before your first visit with us. Use our Insurance Coverage Checklist to assist you with what questions to ask to get accurate information about your coverage. If you don’t have insurance coverage, please see the bottom of the checklist for helpful information.

Our patients need to be referred to us by their primary care doctor or another provider. Your doctor can use this Physician Referral Order form to make the referral.

It will be helpful if you complete our Assessment Form before your first visit so we can provide the care that best fits your needs.
