From maternity and pediatric care to services for seniors, we’ll take good care of your family. The Providence Family Medicine Center provides complete medical treatment with open access for every member of your family.

We meet health care needs for all ages, including care through pregnancy and delivery. We treat bumps and bruises, chronic or acute conditions, and provide services ranging from immunizations to minor surgery. As a certified Patient Centered Medical Home, our clinic integrates primary care and behavioral health, providing patient-focused services from pre-natal through end-of-life care.

You’ll have a dedicated primary care resident or faculty family doctor working closely together to provide complete medical evaluation and treatment. We orchestrate all your care, whether it’s outpatient, inpatient, long-term, home care or residential facility services.

We care for a wide variety of conditions and refer you to specialists as needed. Conditions we treat include:

To schedule an appointment, please call 907-562-9229. If it’s your first visit, please arrive 20 minutes early and bring the following:

  • A completed New Patient Intake Form 
  • Any pertinent medical information
  • Current medications
  • Copy of your insurance card (required for each visit)
  • Payment is expected at the time of service (copays, sliding-scale fees, etc.).

You are considered late if you don't arrive by your scheduled appointment time. We will do all we can to accommodate you, but we may need to reschedule your appointment.

If you need to cancel an appointment, please provide 24 hours’ notice to avoid being considered a “No Show.” Patients with 3 or more documented "no shows" or cancels with less than 24 hours may be charged a fee and be reviewed for possible discharge from the practice.

Financial assistance

Ask us about a financial assistance program that offers reduced rates for qualified individuals based on household size and income. Applications are available at the front desk or by downloading the Financial Assistance form. To qualify for a reduced rate, you’ll need to submit a fully completed form within 10 working days of your visit.

Medical records

To request a copy of your medical record, you will need to sign a Release of Information form and indicate the specific information you are requesting.

The turnaround time is a maximum of 30 days. Medical Records will contact you when your records are available. Your doctor’s office can request a copy of your medical records at no charge.

Your care team provides a full range of services, from education and evaluation to consultation, medical care and follow-up services. Led by a doctor or other care provider, specialists work together to contribute to your care.

Family Medicine Residents

We support the Alaska Family Medicine residency program through the University of Washington. Your doctor may be one of our residents completing a three-year training program specializing in family medicine. Residents receive rigorous training and are medical school graduates. They’re doctors with an MD degree practicing as a resident of a hospital where they receive specialized family medicine training, often focused on rural medicine.

We prepare residents with a solid foundation of medical knowledge, diverse skills and a thorough understanding of the health care system and our community needs. Our residents receive extra training in the following areas to meet the unique needs of Alaska’s rural communities:

  • Obstetrics
  • Orthopedics
  • Pediatrics

  • Transcultural medicine
