Your team

We are a Patient Centered Medical Home. This means we provide patient centered care with a team based approach. Some of our team members available for your care include: PSS, CMA, RN case manager, financial specialist, social worker, pharmacist, behavioral specialist, physician assistants, and our providers.

Nurse case manager: Works with other disciplines to identify patient’s ongoing needs, provide patient education and facilitate reaching various goals.

Pharmacy consultation: Works with the physician to educate and safely manage patient medications.

Behavioral health: Offers screenings, individual therapy, psychological assessment, Behavioral Health Groups, and consultation.

Social work services: Assist patients to prioritize, and address numerous patient concerns, counseling, and linking patients to resources.

Home visits: Home visit providers who take the clinic to the patient at their home or assisted living facility.

Group visits: Voluntary visits designed to improve access for patients (weekly post discharge, diabetes, hypertension, chronic pain management.

Alaska Family Medicine Residency

Did you know that the Providence Family Medicine Center supports the Alaska Family Medicine residency program?

Your physician may be one of our residents completing a 3-year training program to specialize in family medicine. They are supervised by our experienced faculty physicians to provide quality care to our patients incorporating the latest medical knowledge and techniques. The resident physicians are part of your patient-centered medical home team, which also includes nurses, social workers, medical assistants, and administrative personnel.

Insurance/Payment Information

PAMC has a Financial Assistance Program that provides reduced rates for qualified individuals. Various rates are available based on your household size and income. Financial Assistance applications are available at the front desk or downloading an application from. They need to be completed with all required financial information and submitted within 10 working days of your visit to qualify for a reduced rate.

Medical Records

To request a copy of your medical record, you will need to sign a release of information form and indicate the specific information you are requesting. The turnaround time is a maximum of 30 days. Medical Records will contact you when the records are ready to pick up. A physician’s office can request a copy of your medical records at no charge.
