Providence Seward Medical & Care Center offers financial assistance for medical services provided if a patient qualifies. In order to qualify for assistance, the Medicaid application for services will need to be completed and submitted to the address on the application.
A certified copy of your birth certificate will also need to be submitted with this application. To retain a certified copy of your birth certificate and receive assistance with your Medicaid form, please schedule an interview with the Medicaid fee agent at SeaView Community Center at 907-224-5257. If you are currently out of state, please write on the front of the form that you would like to request a phone interview (available through Kenai).
Once you are qualified or denied from Medicaid, please submit your approval/denial letter along with the financial assistance application for Providence Seward Medical Center to:
