Laboratory Client Services

Providence Laboratory Client Services department is staffed by knowledgeable and experienced representatives, trained to offer timely and accurate answers to your inquiries.
Client Service Representatives are available to answer questions regarding:
  • Test information
  • Turn-around-times
  • Test prices
  • Specimen collection
  • CPT codes
  • Transport
  • Patient results
  • Supply Order Form
Client Services also faxes patient reports and fills supply requests. If you require information beyond the scope of the Client Service Department, they will connect you directly with the technical staff person best able to answer your questions.
Courier Service for Physician Offices
Express Delivery Service provides specimen pick-up and supply delivery to our clients in the Anchorage, Eagle River, and MatSu area. If you are not currently on an Express Delivery route, please contact us for details on how you can take advantage of this service.
Outpatient Requisition Forms and Supplies
Requisition forms for clinical laboratory, cytology, and pathology specimen testing are available for your convenience, and will expedite the processing of specimens through our laboratory. Please use these forms when ordering testing.
Customized lab requisitions
Available upon request by calling 212-3631. Providence Laboratory Client Services will provide the forms and supplies necessary to collect and transport laboratory specimens to our facility for testing. To order supplies, please fax a list to 212-3632, or call 212-3631.
