Dedicated Care Team

Providence Medical Group Primary Care offers the NCQA Patient-Centered Medical Home* model of care.

With our medical home model, you and your family have a dedicated team focused on providing you the best care possible. For your convenience, you can see multiple members of your care team during a single appointment; you no longer need to set up multiple appointments at various locations.

Patient care team members:

Physician, nurse practitioner and physician assistant

Each care team includes a board-certified physician, nurse practitioner or physician assistant. Nurse practitioners and physician assistants collaborate with physicians as needed to ensure the highest quality of care.

Clinical pharmacist and pharmacy technician

The pharmacist is located within the clinic, assists with medication reconciliation and, at the request of your provider, may assist with patient medication management and education.

Clinical social worker and health navigator

Social workers and health navigators can facilitate access to other services needed, eliminating barriers to care. They assist you in connecting with community resources, such as transportation arrangements and durable medical equipment, and follow up on referrals.

Psychiatric nurse practitioner and psychologist

Behavioral health is part of our comprehensive care. A psychiatric nurse practitioner is available to diagnose and manage disorders as well as manage medication with a referral from your provider. A psychologist is also available to provide therapy.
